Adirondack Internal Medicine & Pediatrics, PC is recognized as an NCQA level 3 (highest level) Patient Centered Medical Home.

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others.   NCQA is a private, nonprofit organization dedicated to improving health care quality.

Monitoring of clinical quality measures and preventive services to ensure you are up to date with recommended services and standards of care.

Working together, we can improve access, meet quality measures and help to lower overall healthcare costs by promoting preventive services and proactively managing chronic illnesses.   Our goal is simple; to constantly work toward improving the lives and health of our patients.
